Health Check for Students Riding the Bus to SMS

If your child is traveling to SMS by bus, please complete the following health check daily before sending your child on the bus. Students without access to internet will be provided with paper health check forms. For the first day of school, a paper note indicating that a health check has been completed is sufficient. You may have received a form by the transportation department that could be used as well. Your cooperation in helping us ensure the safety of all our staff and students is very appreciated!
